Add-On for registration and not each attendee

I would like to be able to include an "add-on" that is per registration and not asked of each attendee.  Our non-profit does a lot of family tickets where there are multiple attendees but we only need an add-on per family.  One example is at our annual campout where each family can pay extra to include a shelter.  Each attendee does not need to be asked about a shelter in this case.
