Create a Custom Field for a CRM Donation Form

Custom fields are used to collect additional data about a constituent when they submit a CRM form. Some examples include asking what inspired them to donate, as well as asking for a check date, stock sale price, or auction item lot number. The good news is that once a custom field is created, it can be added to any CRM donation form without having to recreate it each time.
Create a Custom Field for a Donation Form
1. Select the Settings cog in the upper-right corner of the page and then select Global Settings in the drop-down menu.
2. Select Donation under the Custom Fields heading.
3. Select New Custom Field.
On the New Custom Field pop-up:
4. Enter an Internal Field Name. This name is used internally in the CRM.
5. Enter a Public Field Label. This is the name that will appear on constituent-facing forms and pages.
6. Select the applicable Display Type for the custom field.
The custom field display types are as follows:
- One-Line Text: This allows the constituent to enter a short-answer response, such as a date or free text, to answer the custom field’s question or prompt.
- Drop Down: This allows the constituent to select a response selection option from a drop-down menu.
- Checkbox: This allows the constituent to select one or more response options from a list of pre-determined response selections.
- Radio: This allows the constituents to select one response option from a list of pre-determined response selections.
- Multi-Line Text: This allows the constituent to enter a longer paragraph-style free text response to answer the custom field’s question or prompt.
- File: This allows the constituent to upload a file as their response to the custom field’s question or prompt.
If you select Checkbox List, Dropdown, or Radio Button, the Options box will appear, as seen in the image below. In the Option Name field:
7. Enter a value option for the applicable list. Select Add Option to add additional value options that will appear in the field option list.
8. When all applicable fields have been completed, select Create Custom Field.
You have now successfully created a custom field for a donation form! If applicable, you may repeat this process to create additional custom fields. When it comes time to create the donation form, you will now be able to see and select this custom field to include it on the donation form.
