Creating a Donation Form Custom Field
To create a custom field for use on a donation form:***
***Please note: You may create as many custom fields as you wish using the process outlined below.
1. Select the Settings gear icon in the upper right corner of the page and select Global Settings on the drop-down menu.
2. Select Donation Custom Fields under the Fundraising heading.
3. On the Account Custom Fields page, select New Custom Field.
4. Enter the applicable custom field question/prompt text in the Field Name field.
5. Select the applicable Account Type. This drop-down allows you to select whether this custom field appears on donation forms for 1) Any account type, 2) Individual account types only, or 3) Company account types only.
6. Select the applicable Display Type. This drop-down allows you to select the type of input field you wish to provide the user to enter their response to your custom field question/prompt. The selections are as follows:

One-Line Text - Allows the user to enter a short-answer response, such as a date or free text, to the question or prompt you provide in the Field Name field. When this display type is selected, you have the option to make a selection on the Data Type field drop-down menu. This optional field allows you to specify the type of information you wish the user to enter in the one-line text field (for example, an integer, an amount of currency, a date, etc.).

Drop Down - Allows the user to select a response selection option from a drop-down menu.
- Example Drop Down Custom Field:

Checkbox - Allows the user to select one or more response options from a list of pre-determined response selections.
- Example Checkbox Custom Field:

Radio - Allows the user to select one response option from a list of pre-determined response selections.
- Example Radio Custom Field:

Multi-Line Text - Allows the user to enter a longer paragraph-style free text response to the question or prompt you provide in the Field Name field. Multi-Line text fields have a character limit of 1024 characters.

File - Allows the user to upload a file as their response to the custom field question or prompt you provide in the Field Name field.

If you selected Drop Down, Checkbox, or Radio in Step 6, continue with Step 7. If you selected any other option, you may skip Step 7 and continue to Step 8.
7. If you selected Drop Down, Checkbox, or Radio in Step 6, a new section will appear, the Available Field Options section (as seen in the first image below). In this section, you must specify the responses you wish to include on the applicable drop-down menu option list, checkbox option list, or radio option list (as seen in the Drop Down menu option list example in the second image below).

To create your Drop Down menu option list, Checkbox option list, or Radio option list:

- By default, only one response field option appears. Select Add as many times as needed to create the number of field option responses needed for the custom field. (For example, if you wish to have four options on a drop-down menu, select Add three times to create four total field option lines.) Note: You may also delete a field option line by selecting the checkbox next to the line you wish to delete and then select Delete.
- In the Field Option field on each line, enter the applicable responses. Note: The Code field is optional and used for internal purposes only. If you wish to include a code for each response selection, enter it in the Code field.
- With all field option responses created, continue to Step 8.
8. The remaining text on the page pertains to legacy donation forms. If you plan to build your donation form using the legacy donation form, follow the instructions provided in this section. For most users, specifically, those using the Form Builder feature, this section does not apply. You may skip this section if you are not using a legacy donation form.
9. Select Save.
10. If you selected the Display Type of Drop Down, Checkbox, or Radio: After you select Save, a page appears on which you may reorder your answer options by dragging and dropping the options into the order you wish them to appear on the donation form. When finished, select Submit.
Your new custom field will now appear on the Account Custom Fields page. When you create your donation form, this custom field will now be available for you to select and include on your donation form.
