Instructions
Case notes allow you to record information about touch points your organization has with a person. For example, your organization worked with a project manager to come up with a version of a case note that you will use. It is up to your organization to decide what should be kept in the note.
Creating Case Notes
- To add a case note, select a record. For this example, the record is located under the Persons tab.
- Select the person’s name within the table.
- Next, select the Case Notes tab within the record.
- Select Add New Case Note.
- Select your name in the Staff Entering Case Note dropdown.
- In the Case Note Information section, today’s date is listed by default. If applicable, enter a Start and End Time. Then, select the type of contact from the Mode of Contact dropdown.
- After, enter a description of your interaction in the Session Description field. You also have the option to upload a file using the Upload File button. Another note you might want to include is the referral. To add a referral, select the Submit and Add Referral button. Otherwise, when finished, select Submit.
- You should see the Case Note in the Case Note table below. You can use these case notes as a reportable history of the work your organization has done with this person.
Note: If you are unable to access this feature in your CCM, it is possible Neon CCM can add the functionality through an upgrade. If this is something you are interested in, have your system’s support contact inquire through a support ticket.
